Use of One Apple health membership constitutes acceptance of the terms and conditions of
One Apple privilege membership when issued, as amended from time to time.

HEALTH MEMBERSHIP
Your health membership is valid for one month or it will be expired after completion of the health check programs.
DELIVERY
Your health package will be sent to you by mail within 14 working days on your day of
purchases. Each program of medical check-up plan is specified per package and can only
be used per clinic visit, implying that each medical items cannot be done separately
or repeatedly.
REFUND
You may change or alter your items of purchase before placing your order.
No refund will be allowed upon purchases after members have agreed and given names
and credit cards for confirmation of orders.
RETURN
One Apple Ltd. is not responsible for any loss or missing package. Should there be
any dispute on missing or incorrect items offered, you are advised to present or
return package or consult our service hotline immediately at 31631234.
REPORT
All medical report will be done by a professional medical team which includes
physician, TCM practitioner, physiotherapist, medical laboratory technologist,
& radiographer & sonographer etc. Should there be any legal dispute of damages
or losses arising from the medical check up programs, all claims should be
directed to the respective centers accordingly.
Under normal circumstances, all medical reports will be ready by 14-21 working days
after booking your consultation appointment from the clinics

Hong Kong : Room 804, Capitol Centre, 5-19 Jardine Bazaar, Causeway Bay, Hong Kong.
All reports are explained over the phone or by direct face to face consultation with doctors.
Reports which do require explanation can be collected by person within 7 working days.
Reports are done by professional medical personnel and it is strictly confidential and
will be issued solely for your personal use. Members are advised not to consider medical check-up
programs as diagnostic tools for treatment purposes
